Action Protocol for Wound Prevention

Document Exact Visual Findings → Notify the Nursing Supervisor Immediately → Contact the Facility Podiatrist or Primary Care Provider → Log Provider Orders and Time Received → Track Outcomes and Changes in Condition

Recognition Precedes Documentation. Documentation Precedes Intervention. Intervention Prevents Hospitalization.

STEP 01

Document Exact Visual Findings

  • Location of wound/skin change
  • Size, color, drainage, odor
  • Pain, swelling, redness, warmth
  • Photograph per facility policy

STEP 02

Notify Nursing Supervisor Immediately

  • Report findings promptly
  • Escalate concerns according to facility protocol
  • Document notification date and time

STEP 03

Contact Facility Podiatrist or Primary Care Provider

  • Provide objective clinical findings
  • Communicate urgency when indicated
  • Document date/time provider contacted

STEP 04

Log Provider Orders

  • Record exact order received
  • Include date and time of receipt
  • Confirm implementation of orders

STEP 05

Reassess, document,
and communicate changes in condition

  • Monitor wound progression
  • Reassess and document findings
  • Record response to treatment
  • Report deterioration immediately
